Everyone who enrolls in health insurance with Surest will receive an ID card mailer, including a welcome letter with Surest registration instructions. The ID Cards are shipped USPS Priority with a 3-5 business day delivery. Digital cards are live on the Surest app and website from the effective date through the plan year.

B. Group identification number
C. Subscriber name and covered dependents
D. Member help information
E. Claims submission information and address
F. Applicable provider network
G. Pharmacy benefits provider and information
