When Anthem receives a claim from your provider, they generate an Explanation of Benefits (EOB) that includes the details of your claim, including how much your plan covered, what you owe and your remaining out-of-pocket balances and more.
You will also receive a bill from your medical provider that should match the amount you owe listed in your Explanation of Benefits. If the bill you received from your provider does not match your Explanation of Benefits, please take a look at this article for steps on resolving the issue.
